***You must either be a Children's Research Foundation Member or have information from your Foundation Member sponsor. To be a member, you must be at VF or above and have submitted an application to NSA. If you are not a Foundation Member, please find out who in your upline is a member and you will need to put their FIN number in the appropriate place on the form. You will receive the retail sales profit, commission, and PVC, but the Foundation Member is responsible for making sure that the customer gets the first survey for the study. Please let the Foundation Member know when you have submitted an order as soon as you put the order in. This is very important as if the Foundation Member is not aware of the customer and does not have enough surveys, the customer's order may not be accepted or might be delayed.
